Entrance Requirements

  • State Certified Birth Certificate

  • Social Security Card / Number

  • Two Proofs of Residency (such as utility bills, driver's license, or WV State car or housing statement) 

  • Photo Identification

  • Immunization Records 

    • DTaP - (Minimum 4)

    • IPV - (Minimum 3)

    • MMR - (Minimum 1-First after 12 months of age)

    • Hep B - (3 doses-Last dose must be after 24 weeks of age)

    • Varicella - (Minimum 1-First after 12 months of age -or- has had chicken pox - proof in writing from physician)
